Job Summary
Evaluate patients' functional status, assess home environments for safety risks, and develop individualized treatment plans focused on restoring independence. Provide therapeutic interventions, fabricate splints, and educate families on adaptive devices and ongoing care needs. Document all patient care accurately within the EMR and collaborate with interdisciplinary teams to coordinate services and support discharge planning. Supervise Certified Occupational Therapy Assistants and oversee home health aide care when therapy is the sole skilled service.
Required Qualifications
- Graduate of an accredited Occupational Therapy program
- Current registration with the National Board for Certification in Occupational Therapy (NBCOT)
- Current state licensure as an Occupational Therapist
- Current CPR certification
- Valid driver's license
- Reliable transportation
- Auto insurance
Desired Qualifications
- Minimum of one (1) year of clinical experience
- Home health experience
