Occupational Therapist (OT) - Home Health (Per Diem) Brooklyn Territory

Perform occupational therapy evaluations in the home, assessing ADLs, IADLs, upper extremity function, cognition, coordination, home safety, and functional independence. Develop individualized treatment plans to improve daily function and quality of life, then train patients and caregivers in adaptive equipment, energy conservation, fall prevention, and safe performance of everyday activities. Monitor patient progress and modify treatment plans as needed while completing timely EMR documentation and collaborating with physicians and the interdisciplinary care team. This independent contractor role offers flexible scheduling and one-on-one patient care for adult and geriatric patients throughout Brooklyn.
