Job Summary
Occupational Therapist (OT) providing in-home, clinician-led care across Marin County and San Jose. Responsibilities include evaluating patient functional status and home environment, identifying equipment needs, supervising Occupational Therapy Assistants, conducting initial evaluations and developing plans of care, documenting progress and updating plans every 30 days, completing comprehensive assessments and OASIS data as indicated, and instructing patients in the use of devices to upgrade independence in activities of daily living. Requires California OT license and Bachelor's degree; in-home evaluations and plans of care are performed with strong documentation and collaboration with Health Care Providers and RNs. Schedule options include Full-Time, Part-Time, and Per Diem.
Required Qualifications
- Bachelor’s degree in Occupational Therapy
- Active California OT license
- Minimum of one year of clinical experience under a licensed Occupational Therapist
