Job Summary

Provide occupational therapy to patients unable to perform daily tasks due to illness or injury, assessing deficits, establishing goals, and documenting progress. Assist patients with exercises to improve fine motor skills and coordination, while suggesting adaptive equipment like grab bars and shower chairs. Evaluate fall risks and introduce strategies to improve home safety. Educate patients and families on the plan of care, exercises, goals, and self-care. Work as part of a home health team including RNs, LPNs, CNAs, and PTs focused on the patient's plan of care and personal goals. This role involves customized therapy in the comfort of the patient's home to help them regain independence and perform everyday activities.

Required Qualifications

  • Graduate of an accredited Occupational Therapy Program
  • Active OT license in the State of Colorado
  • Minimum of 1 year of occupational therapy experience
  • CPR/BLS certification (must be a demonstration course)
  • Knowledge of state and federal home health regulations
  • Good clinical judgment
  • Strong interpersonal skills
  • Resourceful
  • Compassionate
