Job Summary
Conduct in-home occupational therapy evaluations, re-evaluations, and treatment for adult and geriatric patients with orthopedic, neurological, cardiac, or post-surgical conditions. Develop individualized Plans of Care, implement fall prevention strategies, and train caregivers on adaptive equipment and durable medical devices. Fabricate splints, apply manual therapy techniques, and ensure accurate EMR documentation while communicating with physicians and supervising COTAs. This Full-Time or PRN role offers flexible daytime scheduling in Baytown, TX and surrounding areas, ideal for licensed clinicians with one year of home health experience seeking clinical autonomy.
Required Qualifications
- Doctorate in Occupational Therapy from an accredited program
- Active Occupational Therapist (OT) license in the state of Texas or eligible Compact OT Licensure
- Home Health experience (1 year required)
- Current CPR certification
- Valid driver's license
- Reliable transportation
- Current auto insurance
Desired Qualifications
- HomeCareHomeBase (HCHB) experience (preferred)
- Knowledge of home health regulations and Medicare guidelines preferred
