Develop and implement individualized care plans while providing therapy services per physician's orders. Assess functional status, recommend adaptive equipment, and teach new skills or retrain patients in daily activities. Maintain accurate documentation and participate in case conferences. This full-time role offers flexible scheduling for better work-life balance in the East Valley, including Tempe, Mesa, Chandler, and Gilbert.
