Occupational Therapist (OT) Home Health - Dayton

Evaluate patients' condition, function level, and treatment goals to improve their ability to perform daily living and work activities. Design and implement individualized therapy plans, update as needed, and coordinate total patient care with the healthcare team. Provide effective patient care using approved therapy techniques, supervise COTAs and HHAs per care plan requirements, and assist patients with home exercise programs and adaptive equipment needs. Document treatment notes and progress in the patient's chart daily, complete discharge summaries, and submit visit records in a timely manner while maintaining HIPAA compliance. Full-time or part-time role serving Dayton area with paid per-visit rates starting at $65.00.
