Evaluate patients' condition, function level, and treatment goals to design and implement individualized therapy plans that improve daily living and work activities. Provide effective patient care using approved techniques, supervise COTAs and HHAs per care plan requirements, and assist families with home exercise programs and adaptive equipment needs. Document treatment notes and progress in the patient's chart daily, coordinate total patient care with the healthcare team, and complete discharge summaries when needed. Maintain compliance with HIPAA regulations and company policies while submitting all documentation and visit records in a timely manner.
