Occupational Therapist (OT) Home Health- Cincinnati

Evaluate patients' condition, function level, and treatment goals to improve their ability to perform daily living and work activities. Design and implement individualized therapy plans to help patients regain independence in their home environment while coordinating care with the broader rehabilitation team. Provide effective patient care using approved therapy techniques, supervise COTAs and HHAs, and document treatment notes and progress daily. Assist patients and families with home exercise programs and adaptive equipment needs, recommend equipment for the home setting, and complete discharge summaries. Submit patient documentation and visit records in a timely manner while maintaining compliance with HIPAA regulations and company policies.
