Job Summary
Conduct comprehensive occupational therapy evaluations and OASIS assessments in patient homes to develop and modify plans of care. Educate patients and families on therapeutic activities, home safety improvements, and discharge planning while collaborating with physicians, OTAs, and care coordinators. Document visit details, supervise OTAs, and complete required audits and supervisory visits within established timeframes. Manage daily schedules, order supplies, and communicate case changes to ensure continuity of care. This role serves as a clinical resource for staff and students within Carle Health's home health program, with hours varying based on census and program needs.
Required Qualifications
- Bachelor's Degree
- Occupational Therapy
- Licensed Occupational Therapist (OT) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
- Driver's License - Secretary of State (SOS)
- Proof of Auto Insurance - Varies
- Ability to work as a member of an interdisciplinary team
- Valid Illinois Driver's License
- Reliable transportation
