Job Summary
Conduct home health visits for adult and geriatric patients in Stokes and Surry counties, performing thorough evaluations of functional status, muscle function, and ADL performance. Assess cognitive, psychological, and motor aspects to develop individualized intervention plans and evaluate home environments for safety hazards. Identify equipment needs and environmental adaptations to enhance client outcomes, while continually revising care plans in collaboration with nursing. Educate patients and family members on rehabilitative care and activities promoting independent living. Accurately document all observations, interventions, and evaluations using a state-of-the-art touch pad tablet. Requires a current North Carolina Occupational Therapist license, one year of recent work experience, and proficiency with electronic medical records.
Required Qualifications
- A current North Carolina Occupational Therapist license
- A minimum of one year of recent work experience as an Occupational Therapist
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
