Job Summary
Conduct home visits to adult and geriatric patients in Roxboro, Person, Caswell, and Orange counties. Perform thorough evaluations of functional status, muscle function, and cognitive aspects to develop individualized intervention plans. Assess home environments for hazards and identify equipment needs to enhance independent living. Continually revise care plans and educate patients or family members on rehabilitative activities. Document observations and interventions using a touch pad tablet while collaborating with nursing on multidisciplinary care plans. Requires a current North Carolina Occupational Therapist license, one year of recent OT experience, and proficiency with electronic medical records.
Required Qualifications
- A current North Carolina Occupational Therapist license
- A minimum of one year of recent work experience as an Occupational Therapist
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills

- Solid computer skills
