Job Summary
Occupational Therapist in home care evaluates and develops treatment plans for patients of all ages, collaborates with the healthcare team, educates patients and families, documents plans and progress, conducts comprehensive assessments (including OASIS), oversees supervision of OTA and home health aides, coordinates discharge planning and community resources, and ensures safety and home environment optimization to improve functional outcomes and independence.
Required Qualifications
- Bachelor's Degree: Occupational Therapy (Required)
- Licensed Occupational Therapist (OT) - Illinois
- BLS within 30 days - American Heart Association
- Driver's License - Illinois Secretary of State
- Proof of Auto Insurance
