Occupational Therapist (OT) - Home Care

The Home Care Occupational Therapist evaluates and develops treatment plans for patients of all ages to resume or adjust the activities of daily living. Collaborates with health care team and modifies plan of care as appropriate. Serves as a role model in the delivery of professional services and as a clinical resource for staff and students. Reviews plan of care and orders with primary physician, educates patient and family members, documents care with clear next steps, updates goals as patient condition progresses, and coordinates with multiple stakeholders. Performs comprehensive home health OT duties including OASIS assessment, discharge planning, supervision of OT Assistants, and home safety/equipment recommendations to improve function. Ensures regulatory compliance and maintains thorough documentation to support ongoing care and program needs. Job located in Merna, IL with compensation between $34.66 and $59.62 per hour, and relocation assistance is offered.
