Job Summary
The Home Care Occupational Therapist evaluates and develops treatment plans for patients of all ages to resume or adjust activities of daily living; educates patients and families, documents care plans and outcomes, and coordinates with the health care team to ensure timely, compliant care. Responsibilities include reviewing plans of care, educating patients and families, documenting visits with detailed action plans, updating goals as patient condition changes, coordinating with physicians, insurers, and other health professionals, supervising and guiding staff (including OTA supervision), ordering supplies, handling discharge planning, conducting comprehensive OASIS assessments, and ensuring home safety and appropriate equipment recommendations. The role emphasizes evidence-based therapy knowledge, accurate documentation, and adherence to home care regulations; advantageous to have knowledge of OASIS, home health regulations, and Magnet-designated care environments.
Required Qualifications
- Bachelor's Degree: Occupational Therapy (Required)
- Licensed Occupational Therapist (OT) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Basic Life Support (BLS) certification (American Heart Association) within 30 days
- Driver's License - Illinois Secretary of State (ILSOS)
- Proof of Auto Insurance - Varies
