Job Summary
Occupational Therapist (OT) for a full-time position at Concept Rehab serving Skilled Nursing, Long-Term Care, and related care settings. Evaluates patients and develops individualized treatment plans; provides skilled OT services to improve activities of daily living, mobility, cognition, strength, and functional independence; monitors progress and adjusts plans; supervises Occupational Therapy Assistants; collaborates with physicians, nurses, families, and an interdisciplinary team; participates in care plan and discharge planning meetings; educates patients, caregivers, and facility staff; recommends equipment and interventions to improve outcomes; maintains timely documentation and billing; complies with licensure and professional standards. Required Qualifications
