Job Summary
Complete in-home OT evaluations and deliver skilled visits focused on ADLs/IADLs, safety, and functional independence. Build and execute clear plans of care while recommending adaptive equipment and home modifications. Coordinate with nursing and the broader care team when conditions change and document thoroughly in Homecare Homebase. Maintain an active Texas OT license and navigate daily travel across assigned territory. This role supports Adaptive Home Health's mission to improve patient access to high-acuity home care through technology-driven efficiency and autonomy.
Required Qualifications
- Active Texas OT license (in good standing)
- Strong clinical judgment and comfort working independently in the field
- Comfort escalating changes in condition to RN/MD and documenting rationale clearly
- Strong communication and documentation
- Reliable follow-through on orders, frequencies, and patient goals
- Current driver's license and comfort with daily travel across assigned territory
- Able to carry/transport basic therapy equipment as needed
- Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)
Desired Qualifications
- Prior home health experience
- Homecare Homebase (HCHB) experience (or similar home health EMR)
- Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)
- Strong organization and time management
