Job Summary
In-home Occupational Therapist (OT) to complete OT evaluations and deliver skilled visits, develop and execute a plan of care focused on ADLs/IADLs and safety, recommend adaptive equipment and home modifications, coordinate with nursing and care team, and document in Homecare Homebase (HCHB); field-based role in Texas with daily travel across assigned territory and a focus on autonomous patient care.
Required Qualifications
- Active Texas OT license (in good standing)
- Strong clinical judgment and comfort working independently in the field
- Comfort escalating changes in condition to RN/MD and documenting rationale clearly
- Strong communication and documentation
- Reliable follow-through on orders, frequencies, and patient goals
- Current driver’s license and comfort with daily travel across assigned territory
- Able to carry/transport basic therapy equipment as needed
- Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)
