Job Summary
Deliver occupational therapy services in patients' homes, including initial evaluations, follow-up visits, fall risk assessments, and home safety evaluations. Develop and implement individualized treatment plans, assist with interventions, and educate patients and caregivers on adaptive techniques and equipment. Monitor patient progress, communicate changes with the interdisciplinary team, and complete accurate clinical documentation. Work independently in O'ahu while collaborating with a supportive clinical and administrative team to help patients regain independence and improve their quality of life.
Required Qualifications
- Active Hawaii Occupational Therapist license
- Minimum one year of clinical experience
- Active Hawaii Certified Occupational Therapy Assistant license
- Minimum one year of clinical experience preferred
- Strong communication and documentation skills
- Comfortable with community outreach and educational presentations
- Valid driver's license and reliable transportation
- Ability to work independently while collaborating with the interdisciplinary team
Desired Qualifications
- Home health experience preferred but not required
