Job Summary
Deliver occupational therapy services in patients' homes, including initial evaluations, follow-up visits, fall risk assessments, and home safety evaluations. Develop and implement individualized treatment plans, assist with interventions, and educate patients and caregivers on adaptive techniques. Monitor patient progress, communicate updates to the interdisciplinary team, and complete accurate clinical documentation. Work independently with reliable transportation to O'ahu, offering flexible per diem scheduling or an annual salary option based on caseload.
Required Qualifications
- Active Hawaii Occupational Therapist license
- Minimum one year of clinical experience
- Active Hawaii Certified Occupational Therapy Assistant license
- Minimum one year of clinical experience preferred
- Strong communication and documentation skills
- Comfortable with community outreach and educational presentations
- Valid driver's license and reliable transportation
- Ability to work independently while collaborating with the interdisciplinary team
Desired Qualifications
- Home health experience preferred but not required
