Job Summary
Deliver occupational therapy services in patients' homes, conducting initial evaluations, follow-up visits, fall risk assessments, and home safety evaluations. Develop and implement individualized treatment plans, assist with interventions as directed by the supervising OT, and educate patients and caregivers on adaptive techniques and equipment. Monitor patient progress, communicate changes with the interdisciplinary team, and complete accurate clinical documentation. This role offers flexible per diem scheduling with the ability to build your own caseload, featuring one-on-one patient care in O'ahu homes.
Required Qualifications
- Active Hawaii Occupational Therapist license
- Minimum one year of clinical experience
- Active Hawaii Certified Occupational Therapy Assistant license
- Minimum one year of clinical experience preferred
- Strong communication and documentation skills
- Comfortable with community outreach and educational presentations
- Valid driver's license and reliable transportation
- Ability to work independently while collaborating with the interdisciplinary team
Desired Qualifications
- Home health experience preferred but not required
