Job Summary
Deliver occupational therapy services in patients' homes, completing initial evaluations, follow-up visits, fall risk assessments, and home safety evaluations. Develop and implement individualized treatment plans for OTs or assist with interventions for COTAs, while educating patients and caregivers on adaptive techniques and equipment. Monitor patient progress, communicate changes with the interdisciplinary team, and maintain accurate clinical documentation. This role offers flexible per diem scheduling with the option to build your own caseload, providing one-on-one care in O'ahu homes.
Required Qualifications
- Active Hawaii Occupational Therapist license
- Minimum one year of clinical experience
- Active Hawaii Certified Occupational Therapy Assistant license
- Minimum one year of clinical experience preferred
- Strong communication and documentation skills
- Comfortable with community outreach and educational presentations
- Valid driver's license and reliable transportation
- Ability to work independently while collaborating with the interdisciplinary team
Desired Qualifications
- Home health experience preferred but not required
