Occupational Therapist (OT)

Assess, plan, develop, and provide home health plans of care for occupational therapy for patients and responsible caregivers. Identify patient needs for other disciplines and services, then coordinate and evaluate the plan of care with all other disciplines, organizations, and physicians for an assigned caseload. Communicate significant changes in patient status to the physician, rehab coordinator, and appropriate agency personnel when indicated. Demonstrate effective communication skills to maintain open lines between the Lead O.T. Supervisor, Team Managers, and all staff involved in patient care. Requires two years of experience as an occupational therapist and current New York State registration.
