Job Summary

Complete in-home OT evaluations and deliver skilled visits focused on ADLs/IADLs, safety, and functional independence. Build and execute clear plans of care, recommend adaptive equipment and home modifications, and coordinate with nursing when patient conditions change. Document evaluations and visits promptly and thoroughly while maintaining strong clinical judgment and escalating changes in condition to RN/MD as needed. This role supports Adaptive Home Health's mission to improve patient access to high-acuity home care in Texas, utilizing proprietary charting and scheduling technology designed to reduce documentation burden and streamline visit coordination. Full-time Monday–Friday schedule with field-based location.

Required Qualifications

  • Active Texas OT license (in good standing)
  • Strong clinical judgment and comfort working independently in the field
  • Comfort escalating changes in condition to RN/MD and documenting rationale clearly
  • Strong communication and documentation
  • Reliable follow-through on orders, frequencies, and patient goals
  • Current driver's license and comfort with daily travel across assigned territory
  • Able to carry/transport basic therapy equipment as needed
  • Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)

Desired Qualifications

  • Prior home health experience
  • Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)
  • Strong organization and time management
  • Experience using charting systems / EMRs
