Job Summary
Occupational Therapist (OT) needed to evaluate residents, develop individualized treatment plans, and provide therapeutic interventions to maximize independence, function, and quality of life in a skilled nursing/long-term care setting at Mantey Heights Rehabilitation and Care Center in Grand Junction, CO. Responsibilities include comprehensive OT evaluations, implementing patient-centered treatment plans, providing skilled therapy to residents recovering from illness or surgery, assessing and recommending adaptive equipment and environmental modifications, educating residents and families, maintaining accurate documentation, participating in interdisciplinary care planning, monitoring progress, and ensuring compliance with regulatory standards.
Required Qualifications
- Current Colorado Occupational Therapist license in good standing.
