Job Summary
Conduct comprehensive occupational therapy assessments and develop individualized treatment plans to facilitate residents' return to independence and community reintegration. Collaborate closely with clinical caregivers across the integrated care model to ensure timely recovery and full functional restoration for patients in both skilled nursing and outpatient settings. Maintain current National Board certification and state licensure while delivering compassionate, person-directed care aligned with the facility's mission to advance healthcare outcomes.
Required Qualifications
- Graduate of an accredited program of occupational therapy, culminating in a minimum of a bachelor-of-science degree
- Current certificate with the National Board for Certification in Occupational Therapy or eligible for certificate as required by state guidelines
- Valid occupational therapy license in the state where services are rendered
