Job Summary
Occupational Therapist or Occupational Therapy Assistant sought for home health roles in Michigan. Deliver individualized, one-on-one occupational therapy services in patients’ homes, including comprehensive evaluations, development of treatment plans, evidence-based interventions, CMS-compliant documentation, and collaboration with caregivers, physicians, and an interdisciplinary team. Responsibilities include ADL/home safety education, fall prevention, home exercise programs, progress monitoring, updating treatment plans, and communicating changes to the care team. Must hold an accredited OT program degree and a current Michigan OT license; home health experience preferred. Strong clinical judgment, communication, and ability to work independently within a team are required, with reliable transportation and a valid driver’s license. Required Qualifications
- Degree from an accredited Occupational Therapy program.
- Current Michigan Occupational Therapist license in good standing.
- Home health experience preferred (minimum one year).
- Strong clinical judgment, critical thinking, and problem-solving skills.
- Excellent communication, interpersonal, and patient education skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Reliable transportation, a valid driver's license, and current auto insurance.
