Job Summary
Conduct comprehensive evaluations, develop individualized plans of care, and assess ADLs, cognition, strength, coordination, safety, and home environments to restore patient independence. Provide skilled occupational therapy including ADL/IADL retraining, therapeutic exercise, functional mobility, balance, and coordination interventions while educating patients and caregivers on fall prevention and adaptive techniques. Maintain timely documentation and collaborate with physicians and the interdisciplinary team to support successful patient outcomes. Full-time, part-time, and PRN opportunities available in Grosse Pointe, St. Clair Shores, Eastpointe, and Warren areas.
Required Qualifications
- Degree from an accredited OT or OTA program
- Current Michigan OT or OTA license in good standing
- COTA certification, as applicable
- Reliable transportation, valid driver's license, and current auto insurance
Desired Qualifications
- Home health experience preferred
- Strong clinical judgment and problem-solving skills
- Excellent communication and patient education skills
- Ability to work independently while remaining connected to the care team
- Familiarity with Medicare/CMS documentation preferred
