Occupational Therapist

Occupational Therapist at Montello Care Center in Montello, WI. PRN - 2 days per week. Performs initial patient examinations, evaluates referrals, and creates initial plan of care for patients, then revises as appropriate. Delivers and administers occupational therapy under physician orders and plan of care, maintains patient records, and ensures quality outcomes. Works within in-house therapy program with emphasis on Rehab/Skilled Nursing settings; adheres to AOTA standards and state practice acts. Requires licensure in the practicing state and accredited OT education; EMR experience and strong administrative/communication skills.
