Job Summary
Provide one-on-one Occupational therapy to older adults in-home or within senior living environments (private residences, memory care, independent living, assisted living). Deliver evaluations, develop individualized plans of care, and deliver evidence-based treatment while coordinating with an interdisciplinary team. Roles include in-home and community-based visits, telehealth when appropriate, documentation in FOX’s EMR, patient and caregiver education to promote safety and aging in place, and flexible scheduling to manage own caseload. The position offers multiple pathways (Full-time in-home/senior living, part-time, and PRN/FOX Flex) with benefits and opportunities for professional growth.
Required Qualifications
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Occupational Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
