Evaluate and diagnose impairments affecting activities of daily living, cognition, and fine motor skills. Create and implement individualized treatment plans including ADL retraining, adaptive techniques, and cognitive strategies. Set clear goals, educate patients on expectations, and build understanding of their rehab process. Counsel patients and families while maintaining precise documentation for all visits and progress tracking. Collaborate closely in a supportive interdisciplinary team with PT, SLP, nursing, and physicians to prevent complications and reduce readmissions.
