Job Summary
Occupational Therapist role based in Hull within the integrated Adult Social Care service. Deliver strengths-based, person-centred OT assessments to support independence at home; provide equipment fitting, demonstrations, adaptations and maximize use of assistive technology. Offer information and guidance to help build self-care skills, strengthen support networks, access community groups and Connect to Support for information and self-funded services. Help individuals build community connections to reduce isolation and support timely discharge; refer into Social Prescribing where beneficial. Lead or contribute to case management, safeguarding reviews and best-interest decisions; work with colleagues across Adult Social Care and housing partners to ensure adaptations meet assessed needs. Qualifications include HCPC registration as an Occupational Therapist. Benefits include DBS/compliance support, payroll services, and access to professional training.
