Job Summary
Provide one-on-one occupational therapy to older adults in private residences, senior living communities, and via telehealth. Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment supporting mobility, independence, and functional longevity. Document sessions effectively within FOX's EMR system and collaborate with an interdisciplinary team of PTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff to promote safety and aging in place. Create and manage your own treatment schedule with flexibility to adjust hours as needed. Drive to patient locations and communities as part of the Geriatric House Calls model.
Required Qualifications
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Occupational Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
- Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Performing CPR
- Providing moderate to maximum assistance with patient transfers and bed mobility
- Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
- Driving to patient homes and senior living communities as part of the Geriatric House CallsTM model
- Wearing PPE when required
