Assess, plan, and organize rehabilitative programs to restore function in activities of daily living, functional mobility, and cognitive tasks for patients with disease or injury. Implement treatment plans, select appropriate activities, and revise goals with patient and family input while providing ongoing education and discharge instructions. Document evaluations, treatment sessions, and care plans according to departmental guidelines and protocols. Collaborate with physicians to establish and evaluate treatment programs, and act as a referral source for staff, families, and physicians regarding rehabilitation services and equipment. Adhere to all regulatory standards, infection prevention precautions, and Wellstar Health System policies while participating in staff meetings and orientation training.
