Assess, plan, and organize rehabilitative programs to restore function in activities of daily living, functional mobility, cognitive tasks, strength, coordination, and range of motion for patients with disease or injury. Implement treatment plans, select appropriate activities to progress patients toward goals, and revise plans with patient and family input. Provide ongoing education, discharge instructions, and referrals to community resources while documenting evaluations, sessions, and required reports per departmental guidelines. Collaborate with physicians to establish and evaluate treatment programs and goals. Adhere to Wellstar Health System policies, infection prevention precautions, and cost-conscious service standards across the full continuum of care.
