Assess, plan, and organize rehabilitative programs to restore function in activities of daily living, functional mobility, cognitive tasks, and range of motion for patients with disease or injury. Implement treatment plans, select appropriate activities, and revise goals with patient and family input while providing ongoing education, discharge instructions, and referrals. Collaborate with physicians to establish and evaluate treatment programs, document evaluations and sessions per departmental guidelines, and complete required family education and referral documentation. Adhere to infection prevention precautions, dress code, and system policies while participating in staff meetings and training new employees.
