Assess, plan, and organize rehabilitative programs to restore function in activities of daily living, functional mobility, cognitive tasks, and range of motion for patients with disease or injury. Implement treatment plans by selecting appropriate activities, revising goals with patient and family input, and providing ongoing education and discharge instructions. Collaborate with physicians to establish and evaluate treatment programs while documenting evaluations, sessions, and family education according to departmental guidelines. Maintain attendance and punctuality, adhere to dress code and infection prevention precautions, and participate in staff meetings and orientation training. Comply with all Wellstar Health System policies, standards of work, and code of conduct.
