Assess patients, develop treatment plans, and implement activities to restore function in activities of daily living, mobility, and range of motion. Collaborate with physicians to establish goals and discharge plans, while providing ongoing education, discharge instructions, and referrals to community resources. Document evaluations and sessions per departmental guidelines, maintain attendance and punctuality, and adhere to infection prevention precautions. Participate in staff meetings, orientation training, and process improvement activities. Comply with all Wellstar Health System policies, standards of work, and code of conduct.
