Assess, plan, and participate in rehabilitative programs to restore function in activities of daily living, functional mobility, cognitive tasks, strength, coordination, and range of motion for patients with disease or injury. Implement treatment plans, select appropriate activities to progress patients toward goals, and revise plans with patient and family input. Provide ongoing education, discharge instructions, referrals to community resources, and collaborate with physicians to establish and evaluate treatment programs. Document evaluations, sessions, and family education according to departmental guidelines and protocols. Adhere to Wellstar Health System policies, infection prevention precautions, and dress code requirements while maintaining punctuality and attending staff meetings.
