Assess, plan, and organize rehabilitative programs to restore function in activities of daily living, functional mobility, and cognitive tasks for patients with disease or injury. Implement treatment plans by selecting appropriate activities, revising goals with patient/family input, and providing ongoing education, discharge instructions, and referrals to community resources. Document evaluations, treatment sessions, and care plans according to departmental guidelines while collaborating with physicians to establish and evaluate treatment programs. Adhere to all regulatory standards, infection prevention precautions, and Wellstar Health System policies, including mandatory training and participation in staff meetings.
