Assess, plan, and organize rehabilitative programs to restore function in activities of daily living, functional mobility, and cognitive tasks for patients with disease or injury. Implement treatment plans, select appropriate activities, and revise goals based on patient and family input while providing ongoing education and discharge instructions. Collaborate with physicians to establish and evaluate treatment programs, document evaluations and sessions per departmental guidelines, and refer staff, families, and physicians for rehabilitation services and equipment. Maintain attendance, adhere to dress codes and infection prevention precautions, and participate in staff meetings and mandatory training.
