Assess, plan, and organize rehabilitative programs to restore function in patients with disease or injury. Implement treatment plans, select activities to progress patients toward goals, and revise plans with patient and family input. Provide ongoing education, discharge instructions, and referrals to community resources. Collaborate with physicians to establish and evaluate treatment programs, and document evaluations, sessions, and care according to departmental guidelines. Maintain attendance, adhere to dress codes, and participate in staff meetings and process improvement activities. Comply with all regulatory standards, infection prevention precautions, and Wellstar Health System policies.
