Assess, plan, and organize rehabilitative programs to restore function in activities of daily living, functional mobility, and cognitive tasks for patients with disease or injury. Implement treatment plans, select appropriate activities to progress patients toward goals, and revise plans with patient and family input. Provide ongoing education, discharge instructions, referrals to community resources, and collaborate with physicians on evaluation and discharge plans. Document evaluations, treatment sessions, and required paperwork per departmental guidelines while maintaining attendance, punctuality, and infection prevention precautions.
