Job Summary
Assess, plan, and organize rehabilitative programs to restore function in activities of daily living, functional mobility, cognitive tasks, strength, coordination, and range of motion for patients with disease or injury. Implement treatment plans, select appropriate activities to progress patients toward goals, and revise plans with patient and family input. Provide ongoing education, training, discharge instructions, and referrals to community resources. Document evaluations, session protocols, and required submissions per departmental guidelines while collaborating with physicians to establish and evaluate treatment programs. Maintain attendance, adhere to dress codes and infection prevention precautions, and participate in staff meetings and training.
