Assess, plan, and organize rehabilitative programs to restore function in activities of daily living, functional mobility, and cognitive tasks for patients with disease or injury. Implement treatment plans by selecting appropriate activities, revising goals with patient and family input, and providing ongoing education, discharge instructions, and referrals. Document evaluations, treatment sessions, and care plans according to departmental guidelines while collaborating with physicians on evaluation and discharge strategies. Maintain attendance, adhere to dress codes and infection prevention precautions, and participate in staff meetings and orientation training for new employees.
