Assess, plan, and organize rehabilitative programs to restore function in patients with disease or injury, including functional mobility, cognitive tasks, and range of motion. Implement treatment plans by selecting activities to progress patients toward goals, revising plans with patient and family input, and providing discharge instructions and referrals. Collaborate with physicians to establish and evaluate treatment programs while documenting evaluations, sessions, and education according to departmental guidelines. Maintain attendance, adhere to dress codes and infection prevention precautions, and participate in staff meetings and orientation training for new employees.
