Job Summary
Occupational Therapists (Independent Contractors) sought to support a hospital-to-home continuum with in-home and clinic-based interventions. Responsibilities include assessing and supporting patients to maximize independence, conducting in-home assessments, developing individualized collaborative care plans with input from clients and families, and working with an interdisciplinary team (physicians, nurses, social workers, and other professionals). Ideal candidate will have strong clinical reasoning, experience with assessments and treatment planning, and the ability to educate clients and families on adaptive equipment, safety, and environmental modifications. Must be licensed with COTO and hold a recognized OT program credential; Ontario driver's license and reliable vehicle are required. Role supports both in-person and remote practice within a contract framework across community and inpatient-to-home transitions.
Required Qualifications
- Completion of a recognized Occupational Therapy Program
- Licensed with the College of Occupational Therapists of Ontario (COTO)
