Job Summary
Provide one-on-one occupational therapy to older adults in private residences and senior living communities, delivering evidence-based treatment to improve mobility, independence, and quality of life. Perform evaluations, develop individualized plans of care, and document effectively within FOX's EMR system while collaborating with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists. Educate patients, caregivers, and staff on safety and aging in place, incorporating telehealth or virtual care when appropriate to extend access and flexibility. Create and manage your own treatment schedule to support autonomy and continuity of care.
Required Qualifications
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Occupational Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
- Ability to exert medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Ability to perform moderate to maximum assistance with patient transfers and bed mobility
- Ability to reposition patients, stand/walk for extended periods, climb stairs, and perform therapeutic activities
- Ability to drive to patient homes and senior living communities as part of the Geriatric House Calls model
- Ability to wear PPE when required

- PRN / FOX Flex Occupational Therapist – Home & Community Visits (pay per unit compensation)
