Job Summary
Provide occupational therapy evaluations and develop/implement a comprehensive home-based therapy plan of care for hospice patients, collaborating with the interdisciplinary team to evaluate physical, cognitive, and functional status. Educate patients and families on treatments, safety, and home exercise programs; supervise and coordinate services with Home Health Aides; contribute to discharge planning and caregiver education; document assessments and communications per standards; facilitate referrals and ensure compliance with physician orders, Medicare guidelines, and hospice protocols; leverage EHR and collaborate with ministry staff, hospital staff, and community resources to optimize patient outcomes.
Required Qualifications
- Bachelor's or Masters Degree in Occupational Therapy
- California Occupational Therapist License (Required upon hire)
- National Provider BLS - American Heart Association (Required within 30 days)
- 1 year experience in acute care, rehabilitation, or home care
- Home care experience preferred
- Knowledge of Medicare Conditions of Participation and regulatory guidelines
- Proficient in using computers and Microsoft Office Suite; knowledge of Electronic Health Record systems preferred
