Occupational Therapist

Evaluate residents and develop individualized occupational therapy treatment plans based on physician orders. Provide skilled OT services, establish short- and long-term goals, and implement or modify plans as resident progress dictates. Monitor responses to treatment, document progress, and participate in assessments and care plan development. Collaborate with nursing, therapy, and interdisciplinary team members to deliver resident-centered care focused on maximizing independence and quality of life.
